We look forward to your enrollment and can't wait to meet you! At Celebrate Primary Care, we strive for you to reach your maximum health. For that reason, we do not prescribe medications for chronic pain management. We also do not write controlled medications for ADHD, anxiety (ie Xanax, Valium, etc) and pain. 

All new members will pay a one time ENROLLMENT FEE of $100 at the first visit. Regular billing will start the month you enroll regardless of first visit date:

• Children 0-17 years olds*, $45/month with at least one parent enrollment
  *At this time we are not able to provide routine vaccinations, call to discuss how we can help arrange these for you.

• Adults 18-50 years old, $65/month

• Adults 51-64 years old, $85/month

• Adults 65+ years old, $100/month
